 If you were referred to Germane & Wise's services by your primary care provider, you are what we call a "collaborative care patient." 

If you are a patient who has received treatment from Dr. Caroline Fisher one-on-one, you are what we call a "direct care patient." 

Please read on to learn more about how to get care, based on what sort of patient you are.

How Collaborative Care Patients Get Care

If your primary care physician has referred you to Germane & Wise for collaborative care, contact your primary care clinic to get in touch with us. You probably already know our psychiatric assistant in the clinic! Your primary care clinic can help you with scheduling or rescheduling.

How Direct Care Patients Get Care

If you are a direct care patient of Germane & Wise Mental Health (meaning you meet with Dr. Fisher one-on-one), you may use the patient portal below to message your provider directly, schedule or reschedule, and request refills.  

You may also text or call or email Dr. Fisher directly if she has given you her contact information.

If you cannot reach one of our providers, you may also call our main number and they will take a message or try to get your provider on the line (assuming she is able to answer).
